BECOME A FOUNDATIONAL FREEDOM MEMBER

Private Membership Association Agreement

Effective Date: October 1, 2026

This Agreement establishes the terms of voluntary membership in Foundational Freedom, a private membership association.

​

1. Voluntary Membership & Transition

​

Foundations for Health LLC is transitioning the applicable private wellness services, member resources, product access, and communications described here into Foundational Freedom, a Private Membership Association (“Association”).

By signing, the Member voluntarily requests membership and enters a private contractual relationship with the Association. Membership is voluntary and may be ended as provided below.

2. Membership Access & No Membership Fee

​

Accepted membership provides access to: (a) the private booking pathway for available appointments with Dr. Lynne Ward; (b) designated ordering pathways for DesBio and Physica products, subject to availability and applicable ordering requirements; and (c) member-only educational information, wellness resources, announcements, product information, and other communications Dr. Ward or the Association chooses to provide.

There is no purchase required, membership fee, enrollment fee, recurring fee, or other financial commitment required to join Foundational Freedom or sign this Agreement.

Signing does not obligate the Member to purchase products, book an appointment, or participate in a service.

Appointments, products, or optional services later selected by the Member may have separate costs disclosed before purchase or participation.

3. Wellness Services & Member Choice

​

Association offerings may include wellness education, consultations, lifestyle or nutritional information, wellness-oriented services, supplements, products, and holistic practices. Availability may change.

Unless expressly provided by an appropriately licensed professional acting within the scope of that license, Association services are educational and wellness-oriented and are not represented as medical diagnosis, emergency care, or a substitute for necessary care from a qualified healthcare professional.

No particular outcome is promised or guaranteed. The Member remains free to ask questions, decline recommendations or services, and decide whether to purchase or participate.

4. Privacy, Responsibilities & Termination

​

The Association will take reasonable steps to protect member information and confidential communications in accordance with applicable law and its privacy practices.

Members agree to provide accurate information, use member resources appropriately, respect the privacy and safety of others, and

pay any separately disclosed charges for products or services they voluntarily select.

The Member may terminate membership by written notice. The

Association may suspend or terminate membership for material violation of this Agreement, nonpayment for separately selected products or services, unsafe or unlawful conduct, misuse of Association resources, or conduct reasonably determined to be harmful to the Association or its members.

5. Legal Terms, Electronic Signature & Acceptance

​

This Agreement documents a voluntary private association and does not waive or replace any federal, state, or local law, professional duty, or consumer protection that cannot legally be waived.

If any provision is unenforceable, the remaining provisions continue to the fullest extent permitted by law.

The Member consents to electronic records and signatures. By signing or electronically accepting this Agreement, the Member confirms that they have read it, had an opportunity to ask questions and seek independent legal or professional advice, understand that membership itself is free and creates no purchase requirement or recurring financial commitment, and voluntarily agree to become a member of Foundational Freedom.
